Instructions for athletic clearance can be found HERE. In addition, if you have questions, look HERE for FAQ or come see Ms. Winterstein in S200.
Remember to print off the athletic confirmation page and have you and your parent/guardian sign it and turn it in to Ms. Winterstein for final clearance ASAP.

Sports physicals appointment can be scheduled through the BACH community clinic on campus. If this is your first time being seen at the clinic you must fill out one of the forms below.
- Appointment for July 31st
- Appointment for August 7th
- Sports Phyiscial (BACH) forms
- All forms must be filled out and brought to the exam.
